We are pleased to announce the Teamsters’ National Benefit Plan (the “Plan”) is now able to offer direct deposit for all eligible Benefit Plan claim payments.

If you wish to engage in the direct deposit process, please complete the Direct Deposit Application.  The link is provided below.   If we do not receive your request, the Plan will continue to process your benefit claim payment in the usual manner by way of cheque and Canada Post.

Please note, effective June 1, 2018, photocopies of receipts will now be accepted.  Original receipts under the Extended Health provision will no longer be required or returned if submitted.  Furthermore, original receipts must be retained for a period of 12 months in the event the Plan requests verification.

You may also now submit your Direct Deposit and future claims via email:

benefits.pensions@teamstersbenefits.ca

You must ensure you provide clarification in the email subject line in reference to your claim.  For example an Extended Health Claim should read as “Extended Health Claim”.

Please ensure you include a completed Extended Health claim form and attach copies of your receipts.  Incomplete claim forms submitted will be returned.

Should email not be an option, please return your Direct Deposit and future claims via regular mail or fax.
