When the cold and flu season arrives, heading into work or the doctor’s office when you’re sick means you’re potentially infecting others. The short illness claim is designed for such events.
At the discretion of the Plan Administrator, for illness related claims where the Plan’s office has confirmation that a Member has returned to work after 5 calendar days, the physician’s statement will not be required.
Short duration Weekly Indemnity claims, maximum of 5 calendar days, still require the completion of page 1 of the Weekly Indemnity form. The form must be completed in full, under the relevant sections, by the Plan Member and also Employer. As noted, the physician statement is not required. Weekly Indemnity forms which are not completed in full will be returned and may require the completion of the physician’s statement.
